Person Centered Practices (PCP)

What are person centred practices (PCP)?

At Cyfarthfa Park Primary we aim to be person centred in the way we think and teach. We believe that each child is an individual and that learning should be personalised. We use a range of person centred thinking tools during circle time and wellbeing sessions. These tools help staff get to know the children on a deeper level and understand their needs.


PCP Tools includes;

  • Relationship circle

  • What's working/not working

  • Good day/bad day

  • 4 plus 1 question

Using these tools helps form a One Page Profile.


One Page Profiles:

A One Page Profile captures all the important information about a person on a single sheet of paper under three simple headings:

-what people like/appreciate about me,

-what’s important to me

-how best to support me.

What is a person centred review?

With a person-centred review, the child is put at the heart: it's a chance to celebrate the child whilst reviewing their progress in many areas in a positive way.


We hold an annual PCP review for all our children who have ALN. Prior to the review we collect information from the child and everyone who is involved with the child, including parents, any health professionals and school staff using the person centred tools. We find out everyone's different perspectives, what they like and admire about the child and their views on what's working and not working. This is recorded in a step-by-step way usually through an informal meeting in school or remotely via Microsoft Teams.

From that, we vote on the most important areas and produce outocmes and an action plan. In Spetmemebr 2021, these action plans will start to be transferred over to Individual Development Plans (IDPs).
